Req Contact Tel No Of Chinese Regiment Clinic At Which I Purchased Deal.

Hi Req contact tel no of Chinese Regiment Clinic at which I purchased deal. I need to book the treatment and to enquire by when is the tretment expired.   I have googled the name but nothing comes up. I would like to arrange the treatment telephonically as I live about 50 km from the adress given. Also what do I give Regiment as proof . Thanks
