Received Gift Card After Survey- How To Activate?

Took card to store and they said we needed to activate it first. This card came after a survey with a letter saying we had taken a Plan Plus insurance and we did not. We contacted them and was told we did not. What phone number do we call to activate this card or to see how much is on it?
