Can I Modify My Pc By Installing A New Graphics Card In It ?

My PC is getting old with all these new games out there , It's RAM is great and so the processor is even better and so i don't want to get a whole new PC ! its just the graphics card that needs changing , so I was wondering if i could buy a new graphics card and install it to my PC . Could that be possible ? Thank You .
