2006 Chev Trailblazer Refund

I rcvd a notice for a refund for fuel guage repairs for my 2006 chev blazer.  I obtained all the requisite receipts, but now can't find the notice.  How can I obtain a copy of the notice so I can file my claim for costs related to fixing a defective fuel guage??
