Engine Stutters

4.3 l engine the truck stutters when you first get going and then again at 35 45 and 55, if u gas it at first up to 45-50 then the stutter goes away for a bit, getting the code p1345 on the tester. at one point the stutter stopped instead whenever u stopped at a light or sign the engine would rev up (idle high) and act like it was ready to off!! help??
