Behringer Xenyx X1222usb Monitor Send Problem

I've checked all the connections. All good. Mon send output is 1/4". I have a 1/4" to XLR adapter on my XLR cable to monitor which are Active monitors via XLR. Mon fader is maxed with very low volume. Any monitor level on any channel only a very low volume in monitor wedge(s. It's sending a signal, but extremely low sound. No control.
