Insurance Claims

My friend has an Aflac cancer policy and found out last month that she has cancer. What does she do with Aflac next?

Please contact Aflac Headquarters Client Service Call Center at 1-800-992-3522 (1-800-99-Aflac) for immediate service.

Essentially, you just need to submit the claim form, along with the UB04 (hospital bill) and HCFA1500 (non-hospital bill), for prompting review and processing. UB04 and HCFA1500 are standard documents used by all insurance providers for processing claims. They can be obtained via the healthcare provider.
