Did Not Receive 2500 Miles Bonus Card

on flight 300 02/18/2012 ny to lga itineary number U369SL i completed application for a new spirit credit card and was supposed to receive a 2500 mile bonus card.  When i arrived home and looked thru my pappers there was NO BONUS CARD FOR2500MILES

They are supposed to hand it to you or give you a bonus code.
