Please understand that the PUK code is not related to the phone itself but the SIM card that it inserted on the phone. For this, you`ll need to contact your carrier and ask them to unblock it. You can find Tracfone`s Customer Service Number at this link.
Be ready with your phone`s serial number, your account number and phone number as well as the serial number at the back of your SIM card as these information may be required to verify your identity. You can type *#06# on your phone to reveal its serial number (also known as the IMEI) or you can check its box.
