Poor Gas Mileage And Ruff Idle

I have a 1996 mercury Sable 24v dohc, For the past few weeks i have been averaging 8-9mpg a drastic decrease from weeks prior of 14-15mpg. The car is running extremely ruff at start up and at idle. Im also getting a engine code of p0174 bank lean. Mass Air Flow was cleaned no change any suggestions?

Other potential issues: There is a vacuum leak downstream of the MAF; the MAF has become faulty and needs replaced; the bank 2 O2 sensor is faulty; faulty/stuck PVC valve; fuel line issues (i.e. low fuel pressure from plugged/dirty fuel filter, sticking/plugged/failed fuel injector); or an exhaust leak between engine and )2 sensor.
