Follow the instructions below:

First please make sure the phone is connected to WiFi
Then, from the phone, please dial * # * # 8647 # * # * the dialer will be cleared.
Wait about a minute, if the green arch does not appear please reboot the phone.
The green arch should appear after the reboot

Please test out the functionality of the phone by testing out if the phone can make/receive phone calls send/receive SMS/MMS using wifi only. Then try to make/receive calls send/receive SMS/MMS over cellular.If after completing these steps your phone works on WiFi but does not work over cellular please follow the instructions below:

Reboot the phone to make sure it is not still trying to provision the cellular info on the phone. If the issue persists proceed to the next step below.
