Programming Q - Mobile 3rd Party App Directly Posts To Social Media Sites?

I am wondering about 3rd party mobile apps as they relate to social media - I think I know it's possible to connect a 3rd party application (non social media app) in such a way to POST a video or image directly to the user's profile (not necessarily live stream) without leaving said 3rd party application. SO I am hoping to answer these 3 questions specific to each of these major social media sites (Facebook, Insta, Twitter, YouTube, Snapchat, and Periscope) 1) Can it be done/is the API open? 2) How do you do it? (layman's terms maybe?) and 3) Which 3rd party apps are doing it best? I am also wondering how any of this may change if you throw a wearable camera in there that works directly with the 3rd party app...
