Facebook Keeps Ignoring My Request To Add My Middle (maiden) Name On My Page!!

I have made numerous attempts to add my middle (maiden) name to my Facebook account...I have confirmed and followed all of their instructions, yet they continue to ignore my requests...I would like my name to appear on my Facebook page as : Bernie Tieman Shook Perhaps I should deactivate the account, and create a new account? Thanks for your help!
