How Do I Eliminate My Account And The Possability Of Re-activation?

I believe there is a security breach with my Facebook account- I haven't been on in 9 months. I got an e-mail saying my password re-set request went through. I never requested that. I went on and de-activated, but then got an e-mail saying my re-activation was successful. How do I completely eliminate my account and all traces of it. I will never re-activate.
