Membership Expiration Date is for one twelve (12)-month period from the date of enrollment of the primary cardholder. A renewal notice will be sent to the primary member by mail each year and the primary member must authorize renewal or cardholder changes, deletions or additions. When the renewal is processed, membership renewal must be completed for all cardholders on the membership.
