Module Part For Front Headlights.

2005 Trailblazer V6 straight, 4 wheel drive, automatic.  Need to know the name and part number of the module for the front headlights.  My headlights keep going off while driving at night.  And I had a new relay installed.  And it's not the fuses.  Thanks.
