Shits Hard In Drive Reverse And In 1st Gear

I have a 1998 chevy blazer LS 6cyl 4.3 . My car has been shifting great untill today I drove the car to the gas station and when I put my car in drive it did a hard jerk and I pressed the gas and it went slow then jerked again . So I stopped and put it in reverse and it jerked hard again now it shifts gears close together and on the taking off gear it jerks real hard and my car is making a noise when we drove it home slowly . We changed the fluid and screen and it still has this problem . It stated out of the blue. Please help im a sing mom that needs my car . Thank you in advance
