Reason For Surgery.
I had surgery last August. I was sent a questionairre inquiring as to what caused the need for surgery. It got misplaced and my insurance is not paying the facility that the surgery was performed in, due to insufficient information. I would like to provide that info now, so that my claim can be processed.Can I do this online or on the phone?
