What Is My Policy Number

Look at your Aflac policy. You will find the account number listed at the top of the policy.

Look at the wallet card that came with your policy. The wallet card identifies you as an Aflac policy holder. It also contains your account number.

Examine your Aflac billing statement. The account number will be listed at the top of your statement.

Contact Aflac customer service at 800-992-3522 to obtain your account number. In order to obtain your account number from customer service, you must answer questions to authenticate your identity.
