Limitations
How long is to long to fill a claim? I have one that I didnt know I could claim from about a year ago.

Call 1-800-99-Aflac (Aflac policyholder service) to verfiy that your coverage is still active. Explain what you are filing a claim for. Then they can send you the proper form.
Typically, you just fill out the actual claim form, along with submitting the medical records which can be obtained from your health care provider.
Then fax these information (claim form + medical records) to 1-877-44-Aflac (Aflac claims department) for processing.
