I Forgot My Password And Username, What Do I Do ?
I forgot my username and password. How can I retrieve both of them or do I have to create a new account?

You can retrieve your username [user ID] at https://phs.aflac.com/aflac.phs.app/account/forgotlogin. Once that's done, you can reset your password at https://phs.aflac.com/aflac.phs.app/account/forgotpassword.
