Employer Filling Out Forms Takes Forever.
Why do I have to have my employer fill out Aflac forms when the do not pay for it? only I do, it comes out of my check. I need an extension for a surgery and the initial forms took 2 weeks for them to fill out the forms. Can they decline on filing out the forms and fire me also? I am disables and unable to work.

Do you know who is the associate servicing your company's Aflac account? That individual should be acting as your local contact in this claim process.
If you are unable to find out more information about the associate, simply call Aflac Policyholder Service at 1-800-99-Aflac, weekdays 8 am - 8 pm EST. A friendly representative will be able to assist you through the process. Please provide your Social Security number when calling for ID purpose.
